Fostering Conversations: Pelvic Floor to Prostate
Some things can feel too hard to discuss, even with a physician. For instance, how do you bring up the embarrassing need to plan your day around the location of bathrooms or the strain in your relationship because intimacy with your partner is no longer enjoyable? The underlying medical conditions that affect urinary and sexual function range from awkward to life-changing for the millions of men and women who experience them. Under the leadership of Chair Dr. Kathleen C. Kobashi and Vice Chair Dr. Brian J. Miles, the team at Houston Methodist’s Department of Urology is charting ambitious plans to help people address urologic conditions and enhance their quality of life.
Forging Ahead in Men's Urology
While early-stage prostate cancer can be arrested with a prostatectomy, the lifesaving procedure’s unintended collateral damage often impacts men’s urinary and erectile functions — testing the fortitude of their identity and intimate relationships. “That’s when the exaltation of living cancer-free is tempered by a set of new quality-of-life challenges that often brings on anxiety, depression and withdrawal from intimacy,” says Dr. Miles.
With help from supporters in the philanthropic community, Dr. Miles, the Centennial Chair in Urologic Oncology, has begun filling a critical gap in the prostate cancer recovery journey with the introduction of a first-of-its-kind Prostate Cancer Survivorship and Recovery Initiative. As a destination for patients undergoing and recovering from prostate cancer treatment, the initiative will help restore urinary and sexual functions through holistic care. A key goal of the program is to facilitate open communication about sensitive sexual health issues and their effects on the patient’s and his partner’s physical, mental and emotional well-being.
“The program is designed to ensure continuity of care and consistent support through diagnosis, treatment and recovery,” explains Dr. Miles. This continuity lies in sharp contrast to the fragmented system of referrals men often must navigate in their recovery journey.
Research and innovation are also essential components of the program. For example, Dr. Miles has initiated clinical trials that inform the efficacy of stem cells in healing nerves injured during surgery. A natural healing agent, stem cells decrease in number with age. Dr. Miles hypothesizes that replenishing stem cells systemically and/or in the targeted nerves could fully restore function to pre-treatment conditions — and perhaps even improve them.
For 30% of prostate cancer survivors, recovery may include biochemical recurrence (BCR) — rising prostate-specific antigen (PSA) levels that indicate the return of cancer before tumors are detected. As part of the program, BCR care and research will be integrated through vigilant monitoring, tailored treatments and specialized equipment.
Breaking the silence in women's urology
It’s a common misconception that urology is primarily a men’s specialty. In reality, urological issues are very common in women, but they are frequently kept under wraps. All too often, women who experience symptoms associated with pelvic prolapse, bladder and bowel control, and sexual dysfunction dismiss them as inevitable aspects of aging or motherhood, not realizing they are treatable. And for those who suspect there’s a medical concern afoot, it can be embarrassing to bring up — especially when getting care means sitting in a urology clinic waiting room that feels geared toward men. These barriers can delay diagnosis of symptoms, leaving women to suffer in silence, sometimes leading to anxiety, depression or more serious medical conditions that go undetected. Philanthropic support has helped provide the momentum to make the Houston Methodist Urology Clinic for Women a reality — the first and only center of its kind in the Greater Houston area. Launching in 2026, the clinic will be dedicated to elevating and advancing women’s urology care through unparalleled patient experiences, research, and training and education. Dr. Kobashi, the Judith Helmle Shaw and Roy Gordon Shaw, Jr. Centennial Chair in Urology — and one of only seven female urology department chairs in the nation — notes that patients will benefit from the elite clinical team’s personalized and compassionate care delivered in an environment specifically designed and tailored to meet the distinct needs of female patients. “Research plays a starring role in our plans to advance women’s urology,” says Dr. Kobashi. She explains that through the clinic, the Department of Urology will intensify its female-focused urological studies to enhance the effectiveness of treatments, improve diagnostic testing techniques and address a long-running underrepresentation of women in clinical trials.

Summing up the department’s trailblazing plans, Dr. Kobashi adds, “It’s the support of our community that transforms vision into reality. Philanthropic contributions make it possible for us to pioneer urologic initiatives that offer world-class, tailored and holistic treatment options — and, importantly, hope for our patients.”
